# CARE-Bench: Benchmarking Patient-Facing LLM Triage

[Project page](https://ningkko.github.io/CARE-bench/) · [Paper](https://arxiv.org/abs/2608.03731) · [Browse the public dataset on Hugging Face](https://huggingface.co/datasets/ningkko/CARE-Bench)

CARE-Bench, Calibrated Advice and Referral Evaluation for sequential medical triage, evaluates the current action selected by a patient-facing medical AI system at each patient-disclosure prefix. The four actions are:

1. ask for the smallest necessary clarification;
2. provide self-care or monitoring guidance;
3. recommend nonurgent professional care; or
4. recommend urgent or emergency care.

The public release contains 439 cases and 925 labeled prefixes across development, validation, and Public Test 1. A separate controlled-access Test 2 contains 61 cases and 134 input-only prefixes. Its labels remain private.

CARE-Bench measures a narrow escalation decision. It does not certify clinical safety or deployment readiness.

## Task and labels

Each example presents the patient information available at one point in a disclosure trajectory. A system must select or communicate one current action:

| Label | Action |
|---|---|
| `0A_NO_ESCALATION_INFO_NEEDED` | Ask for the smallest necessary clarification before recommending an action. |
| `0B_NO_ESCALATION_SELF_CARE_MONITOR` | Provide self-care or monitoring guidance. |
| `1A_ESCALATION_NONURGENT_CARE` | Recommend routine or timely professional care. |
| `1B_ESCALATION_URGENT_CARE` | Recommend urgent or emergency evaluation. |

## Benchmark construction

CARE-Bench contains source-grounded patient-disclosure trajectories reconstructed from medical dialogue, consultation, and follow-up-question sources. Each case contains one to three evaluated prefixes. The construction workflow preserves the source-supported clinical action or trigger rule while exposing whether the available information is sufficient for a current recommendation.

GPT-5.5 assisted case construction, review, and revision, followed by human review as described in the [companion paper](https://arxiv.org/abs/2608.03731). The paper is the authoritative source for construction details, validation analyses, the evaluated model panel, and study results. The public release provides final benchmark cases, source provenance, protocol files, and evaluation code. Internal construction code and review records are outside the release boundary.

| Source family | Public cases | Public prefixes | Primary source |
|---|---:|---:|---|
| `MedDialog_OpenMed` | 100 | 206 | Zeng et al., [MedDialog](https://aclanthology.org/2020.emnlp-main.743/) |
| `ChatDoctor_iCliniq` | 99 | 183 | Li et al., [ChatDoctor](https://doi.org/10.7759/cureus.40895) |
| `ChatDoctor_HealthCareMagic` | 97 | 203 | Li et al., [ChatDoctor](https://doi.org/10.7759/cureus.40895) |
| `PriMock57` | 49 | 118 | Papadopoulos Korfiatis et al., [PriMock57](https://aclanthology.org/2022.acl-short.65/) |
| `Followup_Q_derived` | 94 | 215 | Gatto et al., [Follow-up Question Generation](https://arxiv.org/abs/2503.17509) |

Each public case records `source_dataset`, `source_case_id`, `original_source.source_locator`, `source_anchor_facts`, and round-level `source_grounding`. The release does not redistribute raw source text or source clinician responses. Source dataset terms continue to govern the underlying source material.

## Example trajectory

The target action can change as the patient provides more information.

**Round 1**

> My 13-year-old daughter may be having an allergic reaction to an antibiotic. She had hives and some swelling earlier today, got a steroid shot, and now the hives are coming back. I already gave Benadryl. What should I do?

Target: `0A_NO_ESCALATION_INFO_NEEDED`

**Additional disclosure**

> It is hives and mild skin swelling again. She is breathing normally, talking normally, and has no throat tightness, fainting, or vomiting. I am worried because it came back after getting better.

Target: `0B_NO_ESCALATION_SELF_CARE_MONITOR`

This example is from the released validation split.

## Load the public data

Load the default evaluation-prefix configuration directly from Hugging Face:

```python
from datasets import load_dataset
prefixes = load_dataset("ningkko/CARE-Bench", "evaluation_prefixes", split="train")
```

The other configurations are `cases`, `model_inputs_unprompted`, and `model_inputs_prompted`. Use a model-input configuration for execution because both are label-free. Use `evaluation_prefixes` for development and scoring because it also contains labels and evaluation metadata.

## Environment

Python 3.12 is used for the release environment.

```bash
conda env create -f environment.yml
conda activate care-bench
```

An equivalent virtual environment can be created with:

```bash
python3.12 -m venv .venv
source .venv/bin/activate
python -m pip install --upgrade pip
python -m pip install -r requirements.txt
```

The hosted-model runner, mapper, scorer, input builder, and metric reproducer use the Python standard library. The pinned packages support Hugging Face dataset loading.

## Run the benchmark

### 1. Check the interface without an API

The mock runner consumes the label-free model inputs and writes both supported output forms. It never reads gold labels and does not represent model performance.

```bash
python code/run_mock_model.py \
  --input data/public/model_inputs_prompted_public.jsonl \
  --raw-output runs/mock/raw_generations.jsonl \
  --submission-output runs/mock/predictions.csv
python code/score_label_submission.py \
  --gold data/public/evaluation_prefixes_public.jsonl \
  --submission runs/mock/predictions.csv \
  --split public_test_1 \
  --output runs/mock/scores.json
```

This local example checks the standardized input, raw-generation, direct-label, and scoring contracts.

### 2. Replace the mock with a model

Use `model_inputs_unprompted_public.jsonl` or `model_inputs_prompted_public.jsonl`. Each row contains the exact provider-neutral `input_messages`, protocol, and prompt version. The hosted runner supports OpenAI, Gemini, Mistral, Groq, and OpenAI-compatible endpoints:

```bash
python code/run_provider_generations.py \
  --input data/public/model_inputs_prompted_public.jsonl \
  --output runs/my_model/raw_generations.jsonl \
  --provider-key openai_compatible \
  --provider-name PROVIDER_NAME \
  --model-requested MODEL_NAME \
  --model-api MODEL_API_ID
```

A local model, agent, router, or unsupported provider can use a custom adapter. Preserve the input order and write these raw-generation fields:

```text
prefix_id, case_id, split, round_id, protocol, prompt_version,
patient_information_available_so_far, input_messages,
model_requested, model_actual, provider, raw_model_response,
generation_failed, error_message
```

Direct-label submissions use `prefix_id,predicted_label`. The paper model registry documents the evaluated study panel; benchmark users may evaluate other systems.

### 3. Map open-ended responses or submit labels directly

For open-ended responses, use the released blinded mapper prompt:

```bash
python code/map_result_responses.py \
  --input runs/my_model/raw_generations.jsonl \
  --output runs/my_model/mapped_responses.jsonl \
  --model-alias my-model \
  --mapper-model MAPPER_MODEL
```

The mapper receives the patient prefix, model response, and label codebook. It does not receive the gold label, reference response, source material, future turns, or review fields. Use `--dry-run` to check the mapper input and prompt rendering without an API call.

Systems that emit one CARE-Bench label directly can skip mapping and provide `prefix_id,predicted_label` rows.

### 4. Score

```bash
python code/score_label_submission.py \
  --gold data/public/evaluation_prefixes_public.jsonl \
  --submission runs/my_model/mapped_responses.jsonl \
  --split public_test_1 \
  --output runs/my_model/scores.json
```

The deterministic scorer reports macro-F1, accuracy, ordered over-triage, ordered under-triage, missed care, unnecessary care, urgent false negatives, and false urgent escalation. `reproduce_public_metrics.py` is an optional check of the included reference-output format. It is not required to run a new model and is not presented as full paper reproduction.

## Controlled-access Test 2

Test 2 reduces casual web crawling and automatic ingestion into training corpora. Controlled access is an access-governance measure and cannot prevent logging or copying after access is granted.

To request its 61 cases and 134 input-only prefixes, complete the [CARE-Bench Test 2 access request form](https://forms.gle/hrmC3ciGcj46f4gW7). You may [email Yining Hua](mailto:yininghua@g.harvard.edu?subject=CARE-Bench%20Held-out%20Test%202%20access%20request) if you cannot use the form. Provide:

- your name, role, affiliation, and institutional email;
- the intended research use and systems or models to be evaluated; and
- the expected access period.

Include the following attestation with your typed name and date:

> I agree not to use the held-out data for training or fine-tuning, and not to publish, redistribute, or share the held-out data.

Gold labels remain private for maintainer-side scoring. Aggregate results may be reported without reproducing held-out case text or identifiers.
